- How We Obtain Consent for SMS: As part of our patient admission process, we include an SMS Consent Form within our admission packet. By signing this form, patients or their responsible parties provide explicit consent to receive SMS messages related to their care. Consent is not required for treatment, and patients can opt-out at any time by texting "STOP" to our number or by informing our staff.
- Purpose of SMS Communications: SMS communications are used solely to facilitate patient care. These communications include:
   - Updates on hospice staff schedules (e.g., staff arrival times).
   - Follow-up on patient care needs, including medications and supplies.
   - Responses to patient or caregiver queries regarding care.
- Opting-Out of SMS: At any time, patients or caregivers can opt-out of receiving SMS messages by texting "STOP" to our designated number or contacting our team. After opting out, you will no longer receive SMS communications but can still communicate with us through other channels such as phone or email.

4. Sharing and Disclosure of Information

We do not share your personal information with third parties for marketing purposes. We may share your information in the following circumstances:
- With healthcare professionals involved in your care.
