The Hospice Care Team

Medical Director

The Medical Director is a doctor whose expertise is resolving pain, shortness of breath, and other symptoms optimally. 

Nurse Practitioner

The Nurse Practitioner works closely with the doctor and RN supporting the delivery of hospice care.  

Registered Nurse (RN)

The Registered Nurse, the lead clinician, plans care and ensures the care plan accurately reflects the person's goals. 

Licensed Vocational Nurse

The Licensed Vocational Nurse is the primary caregiver, ensuring the person receives individualized services, has adequate medications and supplies are home.

Home Health Aide

The Home Health Aide provides compassionate services such as companionship, light housekeeping, and assistance with personal care. 

Social Worker

The Social Worker offers guidance on long-term care, cost of care, advanced directives, and other important matters.

Counselors

Spiritual, bereavement, and grief counselors provide emotional care to the person and their family. 

Therapists

Physical, occupational, and speech therapists work with the person and family to improve their quality of life. 

Dietician

The Dietician offers recommendations that improve a person's dietary intake and optimal nutrition.

"When is hospice the right choice?"

A common misconception is hospice is for the end. A person can receive hospice care as long as their condition continues to meet the medical criteria.


There are 3 instances where hospice care may be the right choice for your loved one. 

When you loved one says ... "I don't want to go back to the hospital."

A trip to the emergency room can be painful, daunting, and costly. The patient is subjected to a battery of tests which often involve painful needles, cold exam tables, and waiting for hours. Concerned families are tasked with arranging transportation to the hospital, being present to speak with doctors and nurses which may mean missing work, and juggling plans to ensure other family members are cared for while their loved one is in the hospital. 


People often go to the emergency room for relief from pain and other symptoms, or to receive treatment for minor injuries. Our nurses and physicians are available 24/7 to provide medical care in the comfort of your home. Whether it's an x-ray, treatment for an infection, pain, or difficulty breathing our team is available to help you avoid the hurt and hassle of going to the hospital. 

When you loved one says ... "There's no cure for my condition."

Despite medical advancements, conditions like Alzheimer's, Parkinson's, aggressive or advanced cancers, and other diseases are incurable. When cure is not an option, comfort becomes the priority.
